Position Description
The Community Health Care Association of New York (CHCANYS) is seeking members to work at their community health centers as a part of the Public Health Corps, with a focus on the COVID-19 pandemic.
Responsibilities include:
- Assisting with COVID-19 vaccine rollout efforts
- Performing community outreach and education about the COVID-19 vaccine
- COVID-19 awareness and education
- Re-engaging patients lost to care
- Assisting with food distribution and increasing food security
- Linking patients and community residents to resources
- Providing emergency preparedness training and education
- Providing other services addressing Social Determinants of Health
- Providing patient navigation, managing cases, and facilitating enrollment
- Conducting screenings for substance use and providing targeted care management services to patients at risk of opioid or prescription drug use disorders
- Providing health education by facilitating health education workshops, participating in health fairs and community events, and conducting one-on-one and group health education sessions with community residents
Position Requirements
- College graduate
- Spanish language skill a plus
Time Commitment
Full-time, with a five to six month commitment.
Compensation
$1,500 per month, plus benefits
